To provide monthly financial assistance to persons with multiple disabilities and mental retardation (intellectual disability) who are above 6 years of age, ensuring minimum financial support for their care and well-being.

Benefits

₹600/- per month is provided as financial assistance.

Eligibility

A. Disability Requirement:

1. Person must have benchmark disability (40% or above)

2. The disability must be certified as multiple disability and/or mental retardation

B. Age Requirement:

1. Applicant must be above 6 years of age

C. Residential Requirement:

1. The applicant must be a resident of Madhya Pradesh

D. Digital Verification Requirement:

1. Applicant’s name must be registered and verified on the Samagra Portal

2. Applicant must also be verified on the Sparsh Portal

E. Income Criteria:

1. No income limit is applicable for this scheme

Application Process (Offline)

A. Collect the application form from the District Social Justice & Welfare Office

B. Fill the form and attach all required documents

C. Submit to the concerned district officer or block-level office

Documents Required

A. Identity & Residence Proof:

1. Native Resident Certificate of Madhya Pradesh

2. Aadhar Card (if required)

B. Proof of Disability:

1. Disability Certificate (must mention multiple disability and mental retardation)

2. Or UDID Card showing the same condition

C. Age Proof:

1. Age Certificate (any official document showing date of birth)

D. Digital Enrolment Proof:

1. Samagra ID

2. Sparsh Portal Verification (as per state guidelines)
