Eligibility

A. For All Applicants (General):

  1. Must be a citizen of India

  2. Must be a permanent resident of Maharashtra

B. For Disabled Employee Applicants:

  1. Must have minimum 40% disability (as specified)

  2. Must be an employee with proven outstanding contribution in any field

C. For Employer / Placement Agency Applicants:

  1. Must demonstrate significant contribution towards hiring or supporting PwDs

Application Process (Offline)

A. Fill the prescribed application form
B. Attach all required documents and bio-data
C. Submit to the District Social Welfare Office or:

  1. Zilla Parishad (for district-level applications)

  2. Assistant Commissioner Social Welfare Office, Mumbai Urban / Mumbai Suburban

Documents Required

A. Identity & Proofs:

  1. Aadhar Card

  2. UDID Card

  3. Residential / Domicile Certificate of Maharashtra

  4. Proof of Age (Birth Certificate / Class 10th or 12th Marksheet)

B. Professional & Recognition:

  1. Proof of Employment

  2. Proof of Outstanding Work

  3. Bio-data

C. Additional:

  1. 2 Passport-sized photographs (signed across)

  2. Details of the Bank Account
