A. Step 1: Visit the District Differently Abled Welfare Officer (DDAWO) during office hours
B. Step 2: Request and collect the application form from the designated staff
C. Step 3: Fill the form carefully with accurate personal and medical details
D. Step 4: Attach all mandatory documents (self-attested, if required)
E. Step 5: Submit the completed application to the District Officer
F. Step 6: Request and retain an acknowledgment receipt with submission details and reference number
A. National Identity Card of the Differently Abled Person
B. Disability Certificate showing relevant diagnosis (Muscular Dystrophy or Tetraplegia)
