Eligibility

A. Hearing-impaired persons with disability above 40% as per RPwD Act, 2016
B. Studying in 6th standard or above
C. Eligible categories:
    1. Students
    2. Employed persons
    3. Self-employed persons

Application Process (Offline)

A. Step 1: Visit the District Differently Abled Welfare Officer (DDAWO) during office hours
B. Step 2: Request and collect the application form from designated staff
C. Step 3: Fill in the required details accurately and attach self-attested copies of documents
D. Step 4: Submit the completed application form to the District Officer
E. Step 5: Obtain an acknowledgment receipt with submission date, time, and reference number

Documents Required

A. National Identity Card of the Differently Abled Person
B. Bonafide Certificate from School Headmaster / College Principal (for students)
C. UDID Card
