A. Step 1: Visit the District Differently Abled Welfare Officer (DDAWO) during office hours
B. Step 2: Request and collect the application form from designated staff
C. Step 3: Fill in the required details accurately and attach self-attested copies of documents
D. Step 4: Submit the completed application form to the District Officer
E. Step 5: Obtain an acknowledgment receipt with submission date, time, and reference number
