B. Children are prepared for enrollment in mainstream education through customized support
A. The beneficiary child must be visually impaired
B. The age of the child must be 6 years or below
A. Step 1: Visit the District Differently Abled Welfare Officer (DDAWO) during office hours
B. Step 2: Request and collect the prescribed application form from the designated staff
C. Step 3: Fill in the form and attach self-attested copies of all required documents
D. Step 4: Submit the completed form with documents to the DDAWO
E. Step 5: Obtain an acknowledgment receipt from the concerned authority with the date and time of submission
A. National Identity Card of the Differently Abled Person
B. Birth Certificate of the child
C. UDID Card
D. Any other document if required by the department
