Provision of Rolator / Walker to children with walking difficulties due to orthopaedic conditions.
A. The applicant should be a child with orthopaedic disability
B. The disability percentage should be 40% or above
C. The child should be facing difficulty in walking
A. Step 1: Visit the District Differently Abled Welfare Officer (DDAWO) during office hours.
B. Step 2: Collect the prescribed application form from the designated staff.
C. Step 3: Fill in all mandatory fields and attach self-attested copies of required documents.
D. Step 4: Submit the completed application form along with documents to the DDAWO.
E. Step 5: Request an acknowledgment receipt with date, time, and unique reference number (if applicable).
