A. Person must have 40% or more disability (Benchmark Disability)

Visually Impaired
Hearing Impaired
Intellectual Disability
Locomotor Disability
C. Age: 5 years and above
A. Step 1: Visit the District Differently Abled Welfare Officer (DDAWO) during office hours
B. Step 2: Collect the prescribed application form from designated staff
C. Step 3: Fill the form and attach self-attested copies of required documents
D. Step 4: Submit the application and documents to the DDAWO
E. Step 5: Request an acknowledgment receipt with date, time, and application number (if available)
