Modified Special Wheel Chairs for Persons Affected with Cerebral Palsy

Details

To provide comfort and easy mobility to individuals affected by Cerebral Palsy through the distribution of modified special wheelchairs.

A. Provision of modified special wheelchairs
B. Aimed at enhancing independence and accessibility for persons with cerebral palsy

Eligibility

A. The applicant must be 12 years or above
B. Must be affected with Cerebral Palsy (Locomotor Disability as per RPwD Act 2016)
C. Must be a resident of the respective district/area in Tamil Nadu

Application Process (Offline)

A. Step 1: Visit the District Differently Abled Welfare Officer (DDAWO) during office hours
B. Step 2: Request the prescribed application form from designated staff
C. Step 3: Fill in the form and attach all required documents (self-attested if required)
D. Step 4: Submit the completed application to the DDAWO office
E. Step 5: Collect an acknowledgment or receipt with details like date, time, and application number

Documents Required

A. UDID Card
B. Date of Birth Certificate
C. Any other documents if required (as informed by the officer)
