Grant Scheme for Purchasing/Repairing of Artificial Limbs and Hearing Aids for Persons with Disabilities

Details

To provide financial assistance for the purchase or repair of artificial limbs and assistive devices to persons with benchmark disabilities, in order to improve mobility, education, and daily life functioning.

Benefits

a. Financial assistance up to ₹15,000/- for purchase/repair of artificial limbs and assistive devices (as per current or revised limits by the Uttar Pradesh Government)
b. For persons with multiple disabilities or those requiring more than one aid/device, a maximum ₹10,000/- in one instance will be granted
c. Eligible equipment includes:
 1. Mobility aids: Tricycle, Wheelchair, C.P. Chair, Crutches, Walking Sticks, Walking Shoes
 2. For visually impaired: Braille kits, geometry kits, abacus, mathematical tools, blind sticks
 3. For hearing impaired: Hearing aids, educational kits
 4. For mentally disabled students: M.S.I.D. kits (Multi-Sensory Education Development Kits)
 5. Daily activity kits (ADL kits) for persons cured of leprosy

Eligibility

a. Must have a benchmark disability of minimum 40% as per RPwD Act, 2016, certified by a competent State Government authority
b. Must be a resident of Uttar Pradesh
c. No age restriction
d. Applicant’s annual family income must fall within the poverty line:
 1. ₹46,080/- in rural areas
 2. ₹56,460/- in urban areas
e. A recommendation from a Medical Officer for the required equipment is mandatory
f. The applicant must not have received such a benefit from Central/State Government or any local body in the last 3 years
 1. However, for students, this limit is 1 year

Application Process (Offline)

a. Application forms can be obtained and submitted at:
 1. Office of the District PWD Empowerment Officer
 2. Jan Suvidha Kendra
 3. Lokvani Center
b. Submitted applications will be categorized and processed on a First Come, First Serve basis
c. If any application is rejected, the reasons will be documented and maintained
d. Decisions of the government regarding scheme disputes are final and binding

Documents Required

a. Recent photograph
b. Domicile/residence certificate
c. UDID Card
d. Caste Certificate (if belonging to SC/BC)
e. Income Certificate
f. Medical Report with recommendation for aids/appliances
