Artificial Limbs and Accessories Delivery Plan

Details

To provide assistive devices and artificial limbs to persons with benchmark disabilities, promoting their mobility, independence, and rehabilitation.

Benefits

a. Provision of artificial limbs and aids such as:

1. Tricycles

2. Crutches (Baishakhi)

3. Hearing aids

4. Braille kits

5. Wheelchairs

6. Tape recorders

7. Calipers

8. White cane sticks

9. Other necessary assistive devices

b. Cost coverage:

1. If income ≤ ₹5000/month: devices provided free of cost

2. If income between ₹5001 – ₹8000/month: devices provided at 50% subsidized cost

3. Maximum assistance value: up to ₹6900 per applicant

Eligibility

a. Must be a resident of Chhattisgarh
b. Must have benchmark disability (40% or more)
c. Monthly income must not exceed ₹8000
d. Applicant should be:

1. From a BPL family, or

2. Recommended by Collector/Sub-Divisional Officer (Revenue)

Application Process (Offline)

a. Applicants must submit a prescribed application form to the Joint/Deputy Director, Social Welfare Department at the district office
b. All supporting documents must be attached

a. UDID Card
b. Disability Certificate
c. Income Certificate
d. Caste Certificate (if applicable)
e. BPL Card or recommendation letter from Collector/Sub-Divisional Officer
