Details

To provide financial assistance to children with benchmark disabilities (80% and above) in Gujarat, to support their care and development.

Benefits

• a) Monthly financial assistance of ₹1,000/-

• b) Paid directly through DBT (Direct Benefit Transfer) by the Gujarat State Government

Eligibility

• a) Disability Requirement:
   1) 80% and above disability as per RPwD Act 2016

• b) Age Limit:
   1) Must be between 0 to 17 years
   2) Assistance discontinues at age 18

• c) Income Criteria:
   1) No income criteria

Application Process (Offline)

• a) Step 1: Visit the District Social Defence Officer or nearest Social Welfare Office

• b) Step 2: Collect and fill the Sant Surdas Scheme application form

• c) Step 3: Attach all required documents (see below)

• d) Step 4: Submit the form for verification and approval

Documents Required

• a) Identity and Proof Documents:
   1) Passport-size photograph
   2) Aadhar Card of the applicant
   3) UDID Card
   4) Proof of Residence

• b) Additional Documents:
   1) Caste Certificate (if applicable) issued by competent authority
   2) Certificate indicating inclusion in BPL list with score up to 20
   3) First page copy of Bank Passbook or Cancelled Cheque
